چکیده

AIM Most dural arteriovenous fistulas (DAVFs) can be treated safely and effectively with endovascular Onyx embolization. Balloon-assisted endovascular Onyx embolization of intracranial DAVFs is especially suitable for arterial protection and arterial flow reduction in complicated DAVFs. We systemically report our experience in the use of arterial balloon for Onyx embolization of DAVF and evaluate the clinical outcomes. MATERIAL AND METHODS Clinical records of 12 consecutive DAVFs patients (five women, seven men) treated with arterial balloon-assisted endovascular Onyx embolization were retrospectively collected to analysis their presenting symptoms, angiographic features, endovascular treatments, and clinical outcomes. RESULTS Six patients presented with cortical venous reflux (CVR). Nine patients underwent transarterial (TA) and 3 patients underwent transvenous (TV) embolization. The balloons were used in 3 types of approach: arterial protection in TA embolization, arterial flow reduction in TA embolization and arterial protection in TV approach. Angiograms demonstrated complete obliteration of the fistula in 11 patients and partial occlusion in one. One patient had Onyx embolization-related complication of transient hemi-facial hypoesthesia. CONCLUSION The Arterial balloon provides protection of intracranial artery and reduction of arterial flow in Onyx embolization of DAVF. This complementary method brought high cure rate.

Endovascular management of intracranial dural arteriovenous fistulas.

Dural arteriovenous fistulas are a heterogeneous group of lesions that comprise 10-15% of intracranial vascular malformations. The treatment strategy is devised after careful consideration of the arterial supply, venous drainage, clinical presentation, and risk of progression, hemorrhage, or neurologic decline. With recent advancements in endovascular technology, the majority of dural arteriove...

Dural Arteriovenous Fistula Presenting with Status Epilepticus Treated Successfully with Endovascular Intervention.

PURPOSE Status epilepticus was rarely reported as the initial manifestation of intracranial dural arteriovenous fistulas. Successful treatment with endovascular intervention has not been reported in the literature. CASE REPORT We report three cases of dural arteriovenous fistulas initially presenting with various types of status epilepticus, including generalized tonic-clonic status epileptic...

Intracranial dural arteriovenous fistulas: A Review

Dural arteriovenous fistulas are fistulas connecting the branches of dural arteries to dural veins or a venous sinus. Digital subtraction angiography remains the gold standard for diagnosing these fistulas. Endovascular treatment is one of the first line options available for their management. This review article reviews the etiopathogenesis, natural history, common classification systems and v...

Objective: To report our experience with the endovascular management of cerebral arteriovenous malformations and dural arteriovenous fistulas. Methods: Fifty patients (66% males and 34% females) with cerebral arteriovenous malformations and dural arteriovenous fistulas were treated in our department between September 2007 and April of 2010 using endovascular therapy. A total of 84 endovascular ...
